1. Introduction

Baseline Group Inc. d/b/a DexaFit Mankato (“DexaFit Mankato,” “we,” “us,” or “our”) respects your privacy and is committed to protecting the personal information you provide to us.

This Privacy Policy explains how we collect, use, disclose, store, and otherwise process personal information when you visit our website, book or purchase services, communicate with us, participate in testing or other services at our location, or otherwise interact with DexaFit Mankato.

DexaFit Mankato is an independently owned and operated licensed operator of the DexaFit® brand and platform. DexaFit, Inc. and its subsidiaries and affiliates (“DexaFit Corporate”) license the DexaFit brand, software, analytics, and related technology and operate portions of the DexaFit software platform used to process assessment data and generate results and reports.

Your use of the DexaFit Corporate software platform, mobile applications, digital reports, or other services operated directly by DexaFit Corporate may also be subject to DexaFit Corporate’s separate Privacy Policy and Terms of Service.

2. Information We Collect

Depending on how you interact with DexaFit Mankato, we may collect the following categories of personal information:

Contact and identifying information. This may include your name, email address, telephone number, mailing address, date of birth, and other information you provide when creating an account, booking an appointment, completing forms, or communicating with us.

Appointment and transaction information. This may include services booked or purchased, appointment dates and times, purchase history, packages, gift cards, promotional offers or codes, and related transaction information.

Payment information. Payments may be processed by third-party payment processors. DexaFit Mankato does not necessarily receive or store your complete payment-card information when payment is processed by those providers.

Assessment and wellness information. Depending on the services you receive, this may include information you provide before or during testing and data and results generated through services such as DEXA body composition assessments, VO₂ Max fitness testing, Resting Metabolic Rate (RMR) testing, and other wellness services provided by DexaFit Mankato.

Assessment information may include body composition, lean mass, fat mass, visceral fat, bone-related measurements, cardiovascular and exercise-performance measurements, metabolic measurements, and other information associated with the services you receive.

Scan imagery and related data. This may include images or visual representations generated during assessments where applicable.

Communications information. This may include emails, text messages, inquiries, feedback, reviews, and other communications you choose to provide to us.

Website and technical information. This may include IP address, browser and device information, pages visited, referring pages, cookies and similar technologies, and information about how visitors interact with our website.

3. How We Collect Information

We may collect personal information directly from you when you book an appointment, purchase a service, complete a form, communicate with us, or receive services; automatically when you use our website or certain online services; from equipment and software used to perform your assessments; through the DexaFit Corporate software platform; from service providers that assist us with scheduling, payments, communications, website operation, analytics, and other business functions; and, where permitted by law, from referral partners or other sources when you have authorized or requested the disclosure.

4. How We Use Your Information

We may use personal information to schedule, provide, and manage appointments and services; conduct DEXA, VO₂ Max, RMR, and other assessments; process assessment information and generate or provide results and reports; maintain customer and transaction records; process payments and administer packages, promotions, gift cards, and refunds; communicate with you regarding appointments, results, services, and customer-support requests; send marketing communications where permitted and consistent with your communication preferences; operate, maintain, secure, and improve our website, services, and business operations; perform quality assurance and staff training; detect or prevent fraud, misuse, security incidents, or unlawful activity; comply with applicable laws, regulations, legal processes, and governmental requests; and establish, exercise, or defend legal claims.

5. DexaFit Corporate and Assessment Data

Certain assessment information may be transmitted electronically to the DexaFit software platform operated by DexaFit Corporate for purposes such as processing assessment data, generating results, producing reports, displaying results, and providing related platform functionality.

DexaFit Corporate’s handling of information within systems it operates is subject to DexaFit Corporate’s applicable Privacy Policy and Terms of Service.

DexaFit Mankato and DexaFit Corporate are separate entities, and DexaFit Mankato is independently owned and operated.

6. How We Share Information

We may disclose personal information when reasonably necessary to operate our business and provide services, including to DexaFit Corporate for operation of the DexaFit software platform, processing of assessment information, generation and delivery of reports, software support, analytics, and related services.

We may also disclose information to testing equipment and technology providers where necessary for equipment operation, software functionality, support, maintenance, troubleshooting, or processing associated with testing; scheduling, website, and technology providers that assist with business operations; payment processors; communications providers that assist in delivering authorized email, SMS, or other customer communications; and professional advisers or governmental or legal authorities when reasonably necessary to comply with law, respond to legal process, protect our rights or safety, investigate suspected unlawful activity, or obtain professional services.

We may disclose information in connection with a merger, acquisition, financing, sale, restructuring, or transfer of some or all of our business or assets, subject to applicable law.

DexaFit Mankato does not sell customer personal information, health or wellness information, or assessment or testing data.

7. Research, Studies, and De-Identified Information

DexaFit Mankato does not independently use customer personal information or testing data for its own research program.

DexaFit Mankato may provide testing or assessment services for specifically approved studies or study participants when appropriate. Information associated with such studies will be handled in accordance with applicable study requirements, authorizations, consents, agreements, and law.

Information may also be de-identified or aggregated where permitted by law and applicable agreements so that it cannot reasonably be associated with an identifiable individual.

8. Email and Text Communications

If you provide your email address or mobile telephone number and consent to communications, DexaFit Mankato and, where applicable, DexaFit Corporate may send communications relating to appointments, services, follow-ups, wellness information, promotions, discounts, events, referral or loyalty programs, or other relevant communications.

Consent to receive marketing text messages is not a condition of purchasing goods or services. Message frequency may vary, and message and data rates may apply.

You may opt out of marketing text messages by replying STOP to a text message. You may unsubscribe from marketing emails by using the unsubscribe link included in those emails.

Even after opting out of marketing communications, you may continue to receive non-marketing transactional or service-related communications necessary to administer appointments, purchases, or services.

9. Cookies and Website Technologies

Our website and service providers may use cookies and similar technologies to operate the website, remember preferences, understand website usage, measure performance, and support analytics and marketing activities.

Some of these technologies may be provided by third parties. Your browser or device may allow you to limit or disable certain cookies, although doing so may affect website functionality.

Where applicable law provides additional privacy rights, DexaFit Mankato will process qualifying requests in accordance with applicable law.

13. Minors and Children's Privacy

DexaFit Mankato’s services are generally intended for adults age 18 and older. A minor may receive services only with the involvement and consent of a parent or legal guardian and in accordance with applicable law and DexaFit Mankato policies.

Our website is not directed to children under 13, and we do not knowingly collect personal information online directly from children under 13 except as permitted by applicable law and with appropriate parental or legal guardian involvement where required.
